LGD-4033

The cleanest trial record here, and the widest dose gap

What circulates
10 mg a day for men over 8 to 10 weeks, up to 15 for experienced users
What was studied
1.0 mg a day for 21 days, and 2.0 mg a day for 12 weeks
The gap
Convention runs above the studied dose
How long it lasts
I could not find a published human half life in the material behind the class page. The dosing in both trials was once daily.

What it actually is

LGD-4033, also called ligandrol and registered in one trial under the company code VK5211, is a non-steroidal compound acting on the androgen receptor. It has the cleanest human trial record of the eleven: a placebo controlled phase 1 in healthy men, and a phase 2 in people recovering from a hip fracture that posted real statistics. It holds no medicines approval anywhere the searches looked.

What it is supposed to do

It acts selectively on the androgen receptor. In the 21 day phase 1 what moved was lean body mass, up an average 1.21 kg at the top dose, while leg press strength, stair climbing and appendicular muscle mass did not separate from placebo.

What people take it for

Muscle gain, and recovery after injury. The hip fracture trial is the closest thing to that second use, and its own registry record carries no functional secondary outcome at all, so in people recovering from a broken hip nothing about function was registered to be measured.

The dose question

What circulates
10 mg a day for men over 8 to 10 weeks, up to 15 for experienced users
What was studied
1.0 mg a day for 21 days, and 2.0 mg a day for 12 weeks
A phase 1 in 76 healthy men and a phase 2 in 108 hip fracture patients

Above the studied dose

Five to fifteen times the top dose of either trial. The 21 day phase 1 randomised 76 healthy men to a dummy or to 0.1, 0.3 or 1.0 mg, and the 12 week phase 2 in people recovering from a hip fracture topped out at 2.0 mg. The guide page carrying the 10 to 15 mg figures also tells readers that clinical trials support up to 222 mg a day for 14 days; its citation list has two entries, and the only trial among them is the 21 day study whose highest dose was 1.0 mg. The one published case report that gives a daily amount describes severe liver injury at 10 mg.

Reported because it is what people use. Nothing here recommends any amount.

How long it lasts

I could not find a published human half life in the material behind the class page. The dosing in both trials was once daily.

Route studied

By mouth, once a day. The phase 1 randomised 76 healthy men aged 21 to 50 to a dummy or to 0.1, 0.3 or 1.0 mg for 21 days. The phase 2 randomised 108 people aged 65 and over to a dummy or to 0.5, 1.0 or 2.0 mg for 12 weeks.

Route used

By mouth, at 10 mg a day for men over an 8 to 10 week cycle and up to 15 mg for experienced users, reported as convention and not as a recommendation.

How to check you have the right molecule

The names ligandrol, LGD-4033 and VK5211 are one compound, and the last is the code the only registered trial runs under, which is why a registry search for the common name comes back thinner than the record actually is. In the 2016 product analysis, LGD-4033 was one of the compounds most often found in products, including in bottles that did not name it.

Walked through on two real certificates in how to read a certificate of analysis.

What people get wrong

The trial doses and the market doses are not the same order of magnitude, and one guide page bridges the gap with a sentence that does not survive checking: it tells readers clinical trials support up to 222 mg a day for 14 days. Its citation list has two entries, and the only trial among them is the 21 day study whose highest dose was 1.0 mg.

What is known about harm

Both halves belong together. In the 21 day phase 1 neither of the two liver enzymes on a routine blood test changed significantly at any dose; what fell instead was HDL cholesterol, triglycerides, total testosterone and the protein that carries it, all in proportion to dose and all back to baseline after the men stopped. That is the only measured recovery of the body's own testosterone the class page research found for any of the eleven. Against that, two published case reports name this compound on its own in severe liver injury, one of them at 10 mg a day, and a third describes it alongside RAD-140.
